We require a valid, current insurance certificate before any trailer is delivered, naming Flex Fleet Trailer Leasing as additional insured and loss payee for physical damage. Our requirements:
  1. Minimum $1,000,000 Combined Single Limit (CSL) liability coverage
  2. Physical damage coverage — or enroll in Flex Fleet's CDW program instead
  3. Flex Fleet Trailer Leasing named as additional insured (liability) and loss payee (physical damage)

The information provided in this application is true and complete. Flex Fleet may receive from and disclose to other persons, including credit reporting agencies, information about the applicant's and/or business owner's account and credit experience, and the applicant and/or business owner authorizes any such person to release credit experience information to Flex Fleet Trailer Leasing.

In consideration of being allowed the privilege of an open credit account, the undersigned — an authorized representative of the company applying for credit — agrees to and accepts Flex Fleet Trailer Leasing's Standard Terms and Conditions in full. In case of default in payment, Flex Fleet Trailer Leasing may, in addition to any other right permitted by law: assess late fees and/or interest; submit the debt to a collection agency; report the debt to major credit bureaus; and recover the costs of collection, including reasonable attorney's fees. This agreement is governed by the laws of the State of California.
